Annual Foundation Awards/Scholarships
WAPA-F High School Senior Award - Deadline March 1st
A $1000 award will be granted to two graduating high school
seniors who are children of WAPA members and are interested
in pursuing a post-secondary education (not necessarily
a medical career).

3 Dr. Ben Lawton Scholarships Didactic Year(s)
- Deadline September 1.
A $1000 Scholarship in memory of Dr. Ben Lawton who was
an early supporter and proponent of the PA profession
in Wisconsin is given to a didactic year student from
each of Wisconsin's three PA programs at the annual Fall
CME conference. It is based on financial need, taking
into consideration academic standing, work ethic and integrity.
The WAPA-Foundation Ben Lawton Scholarship is decided
by the WAPA-Foundation board of trustees.

3 Service Awards Clinical Year - Deadline March
1.
The $1000 Student Service Award will be presented to a
final year (clinical year) PA student from each of the
three Wisconsin PA Programs showing exemplary service
to their community (broadly defined as state, local and/or
campus). The award will be presented at the Spring CME
Conference. The application process includes a personal
statement listing and describing the depth and time of
participation (service from High School to present); along
with two letters of recommendation. Applications will
be accepted December through March, with a March 1 deadline.
The WAPA-Foundation PA Student Service Award is decided
by the WAPA-Foundation board of trustees.

3 Leadership Awards Clinical Year - Deadline
September 1.
A $1000 award is given at the Fall CME conference to one
final (clinical) year student from each of the three programs
who has demonstrated outstanding leadership. Leadership
is broadly defined as leadership in the community, profession,
program or campus. The Leadership Awards are chosen by
each PA programs faculty.
The Robert T. Cooney M.D. Scholarship Recent Graduate
- Deadline September 1
The Robert T. Cooney M.D. Scholarship is an award of $500
to celebrate his life and commitment to the PA profession.
Dr. Cooney practiced family medicine in Portage, Wis.
for five decades. The scholarship opportunity is for a
recent graduate of one of the three Wisconsin PA Programs
who will practice Family Medicine in a small town or rural
setting (population of <10,000). Applicants must be
WAPA student members, and submit a personal statement
of less than 500 words, a letter of job commitment/offer
from their employer, and one letter of recommendation.

Special Projects Grants
The WAPA Foundation encourages submissions for funding
of any special projects that meet its mission of leadership,
education, research or service. Requests most likely to
be funded will not be in excess of other scholarship amounts
or less. Grant requests may be made by any WAPA member
or on behalf of other organizations by a WAPA member as
sponsor. Requests may be made at any time but will not
be voted upon until the next quarterly meeting of the
Foundation Trustees. These meetings occur in conjunction
with Fall and Spring CME conferences, and the January
and July WAPA board meetings. Funding decisions will be
made on the basis of how well the request meets the foundations
mission.

PA of the Year Award
Policy

Purpose
of Policy
To establish an Academy sponsored award honoring outstanding
WAPA members.
Policy
The WAPA will publicly recognize that physician assistant
in the state of Wisconsin who has demonstrated an exemplary
commitment to the physician assistant profession through
the WAPA PA of the Year Award. This person will have demonstrated
a high degree of professionalism in the workplace and
excel in efforts to benefit their patients, community,
and state.
Responsibilities

Procedure
-
Any
WAPA Fellow or Member may be nominated by a peer, Fellow,
or Member. Nominees must be a member of WAPA in good
standing during the year of consideration for the award.

The
application form will be provided upon request from
the administrative office.
-
The
application form must be completed and all additional
information such as letters of recommendation, newspaper
articles, and documentation of services must be postmarked
by the application deadline to be considered by the
selections committee.
-
Members
of the selection team are not eligible for the award.
Should a member of the selections committee be nominated
and chooses to apply, a replacement committee member
will be appointed.
-
Once
a person has been chosen PA of the Year, s/he is no
longer eligible for consideration for five years.
-
A
notice calling for nominations will appear in each Spectator
from May to July. The notice is to include a contact
person and deadlines for nominations. It is the membership
chair's responsibility to be sure the editor includes
this information.
-
Deadline
for nominations is August 15.
-
Identify
selections committee members by September 7. The selections
committee should consist of the current WAPA president,
Membership Chair, the most recently elected Director,
and two Fellows or Members not currently on the board.
These two members of the selections committee will be
recruited and selected by the membership chair. This
is not necessary if there is only one nominee or applicant.
-
The
deadline for all application materials is September
2. Materials must be postmarked by this date.
-
Application
materials are to be sent to selections team members
by September 9 along with matrixes that they will use
to score the applicants as objectively as possible.

Selections
team members must send matrixes to the membership chair
by September 16th. The membership chair will contact
the administrative office as soon as possible after
the scores are added and a winner has been chosen.
-
The
award winner is to be notified by September 23.
-
The
PA the year for the previous year should be contacted
to send the traveling plaque to the administrative office
for engraving by September 15. As soon as possible after
receiving the plaque, the administrative office will
send it in for engraving for the current year.
-
At
the time the traveling plaque is being engraved an individual
plaque should also be engraved.
-
A
presenter from WAPA's board should be chosen to present
the award in the recipient's locale on October 6 if
he or she wishes and arrangements can be made. These
arrangements should begin at the time the recipient
is informed and completed as soon as possible.

A
presentation is also to be made at the Fall CME conference.
The recipient may bring a guest to the luncheon.

A
letter will be generated by the membership chair to
all other nominees.
